perry farrell jukebox 45???

after looking through the discography as thoroughly as i could; it seems i have an official (?) release that somehow flew under the radar.
perry farrell 'song yet to be sung' b/w 'happy birthday jubilee' ,white label black vinyl big hole 45 rpm 7". no pic sleeve, came in a blank white sleeve. says 'for jukeboxes only!' on the left side of the label.
release date: ???
released by: emi-capitol music special markets, also has the virgin records logo on it (i take it emi-capitol is owned by them or vice versa?).

We've got the jukebox single listed on the regular SYTBS single page, but have a really shitty small picture of the vinyl. Any chance you could get a better one?
